Whether you are concerned about their ability to live safely independently, are considering caregiving in your home, or a move to or from a community-based care setting, it is best for everyone involved to take the time to step back and consider all of the implications.
Initial Consultation – A free 15-minute call to determine if CareTreks’ services are a fit.
Intake Call – This 45-60 minute call is with your family’s designated “point person”. We will go over some general health questions (physical & cognitive), options being considered by your family, and basic financial overview. We will also prioritize the topics to be covered during the Family Zoom.
Family Zoom – This 90-minute Zoom conversation with up to 8 decision-making family members takes an objective look at options for your loved one's continued care. The purpose is not to encourage or discourage a particular plan, but to look at all aspects of care, family impact, financial concerns, physical and cognitive challenges, and more in order to set your family and your loved one up for success.
Summary Email - A recap of our conversation, recommendations, links, and referrals will be sent to those participating in the Family Zoom within 2 business days.
Follow-Up - It is often helpful to have an additional 30-minute call (or Zoom) to address new questions or offer clarification. To be scheduled within 30 days of the Family Zoom.
The $650 flat-fee for this package will be invoiced after the Initial Consultation to be paid before the Intake Call.
